Privacy Policy

Effective date: [Effective Date]

1. Introduction

Restored Hope Recovery (“we,” “us,” or “our”) respects your privacy and is committed to protecting the personal and health information you share with us. This policy explains what information we collect, how we use it, and the rights you have regarding that information.

2. Information we collect

This may include:

  • Contact information you provide, such as name, phone number, and email address
  • Information submitted through our admission inquiry and insurance verification forms
  • Health information relevant to treatment, protected under HIPAA and, where applicable, 42 CFR Part 2
  • Website usage data collected through cookies and analytics tools (see our Cookie Policy)

3. How we use your information

We use the information you provide to respond to inquiries, verify insurance benefits, coordinate admission and treatment planning, and, where you’ve consented, follow up by phone, text, or email. We do not sell your personal information.

4. HIPAA and 42 CFR Part 2 protections

Health information related to treatment is protected under the Health Insurance Portability and Accountability Act (HIPAA). If you receive treatment for a substance use disorder, additional federal protections under 42 CFR Part 2 restrict how that information can be disclosed, generally requiring your written consent before it’s shared outside of our clinical team, even with other healthcare providers. Our full Notice of Privacy Practices, provided separately at admission, describes these protections in detail.

5. Cookies and website data

Our website uses cookies and similar technologies as described in our Cookie Policy. Website analytics data is used to understand site usage and improve content, and is handled separately from protected health information submitted through our forms.

6. Third-party services

We may use third-party services to process form submissions, verify insurance benefits, or power website features such as our AI triage chat. These vendors are contractually required to protect any information they handle on our behalf. Vendor names and data-handling terms will be listed here once integrations are finalized.

7. Your rights

You have the right to request access to, correction of, or deletion of your personal information, subject to legal and clinical record-keeping requirements. To make a request, contact us using the information below.
